This notice applies to chiefcornerstonetherapy.com, its forms, and related administrative communications. It describes website and SMS privacy practices; it does not replace the practice's clinical Notice of Privacy Practices or separate treatment agreements. Information you choose to provide
The consultation form collects contact and scheduling information so Sandra can respond to your request. Please do not send diagnoses, treatment records, or sensitive clinical details through the website.
How information is used
Information submitted through the form is used only to respond to your inquiry and discuss scheduling. Website email is routed to Sandra Shields at Chief Cornerstone Therapy Services.
Security and emergencies
HTTPS encrypts information while it travels between your browser and the website. No internet form can guarantee absolute security. This website is not monitored for emergencies; call 911 or call/text 988 for urgent crisis support.
SMS consent and your choices
If you separately opt in, Chief Cornerstone Therapy Services may send text messages about appointment scheduling, reminders, and non-emergency administrative matters. Message frequency varies. Message and data rates may apply. Consent is voluntary and is not a condition of purchasing services or receiving care.
You may opt out of SMS at any time by texting STOP to (832) 802-8027. After an opt-out, no further program texts will be sent except a confirmation of your request. We do not sell or rent personal information. Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes. SMS opt-in data and consent will not be shared with third parties for their own use. We may provide the information necessary to service providers that help deliver our messages and maintain consent records, solely to perform those services on our behalf.

Our text messaging opt-in form collects your name, mobile number, affirmative consent, the exact consent wording and version, submission time, and source page. We use this information to document and honor your communication choices. Consent records are sent to the practice through its email service and retained as needed to administer your choices and meet applicable recordkeeping obligations.
The form uses a temporary session cookie to protect submissions and display errors or confirmation. Hosting and email providers may process information as necessary to operate these services. Contact us to request correction of your information or to withdraw consent.
SMS is not a secure clinical communication channel. Messages may be visible to others with access to your phone. Please do not send diagnoses, treatment details, or other sensitive health information by text. This website policy does not replace separate clinical privacy notices or informed-consent documents.

Information may be processed or stored by service providers that help operate our website, hosting, email, and messaging services. We limit disclosure to what is needed for those services and require appropriate confidentiality and security protections for information entrusted to them. These providers are not authorized to use SMS consent or mobile information for their own marketing.

The website loads typefaces from Google Fonts. Your browser contacts that provider to request font files and may transmit technical information such as your IP address. The website code does not currently include advertising pixels or behavioral advertising analytics. We will update this notice if relevant website practices change.

Children under thirteen
The website forms are not directed to children under 13. Children under 13 should not submit personal information through them; a parent or guardian should contact the practice instead. If you believe a child under 13 has submitted information, contact Sandra so we can address it. Services for minors involve separate intake and consent arrangements.
Email communications
We may email you to respond to your inquiry or coordinate requested services. Contact us to update your preferences. Choosing SMS does not enroll you in email marketing, and withdrawing SMS consent does not prevent you from requesting contact by phone or email.
Changes to this notice
We may update this notice as our services or practices change. Revisions will appear on this page with an updated date. We will provide additional notice when required by law and obtain new consent when necessary. A policy update does not expand the scope of your existing SMS consent.
